There is only a health insurance requirement for a non OA one year visa (or OX). If you get a non O from overseas or convert an exempt or tourist entry visa in country there is no insurance requirement. Some embassies (especially the US) may insist on insurance to issue the visa, but that’s a specific rule for that embassy, not an entry requirement.
