What are the requirements for the guarantor's contact details and medical certificate for a Thai retirement visa application in Australia?

Hi everyone,

I'm just about to post my retirement visa application to the Thai embassy in Australia and during the final check, before taking it to a JP, I'm concerned about the following and wondering if anyone may be able to clarify:

1. Question 18 asks for the contact detail for my local guarantor. I'm assuming this does not relate to my visa?

2. It is stated that I need to have a 'rubber stamp' from my doctor for the medical certificate. He has included his medical licence number which I thought would be sufficient. Do you believe an actual rubber stamp is required also or is that just another way of saying that it needs to be authorised/validated with a licence number?

Can depend on their mood for Q2. I had a document (not for retirement visa) state a stamp, but ID number was written instead and the Thai Embassy in Australia accepted it.
