You can't take a rapid test to fly into Thailand. You must have an RT-PCR test. No QR code needed. Passport number is fine but not needed either, as long as it has your name and birthday that match your passport.

It checks the boxes. It says RT-PCR and has your name and passport number. As long as the results date is within 3 days of your flight, you should be fine.
