If you are over 50 and come to Thailand with a 45 day visa exemption can I apply for a non-O Visa at immigration in Phuket?
I heard you need to apply for a non-O visa at a Thai consulate outside Thailand.
The non-O Visa is for 90 days and can than be stamped for 1 year extension retirement. Do you need for that a medical certificate?

You heard wrong, You can apply for a 90 day Non-O visa at most immigration offices as long as you meet the requirements and as long as you meet the proof of funds
There is no medical certificate required to get the in country 90 day Non-O visa or the year extension after that (except at Samui I believe ๐ )
The important thing is that you have enough time left on your entry stamp to apply and wait out the under review period after you pay the 2000baht for the 90 day Non-O visa As it says on the link I jumped you
In Phuket they require a minimum of 21 days left on the current entry permission stamp to process the Non-O application.
