My retired parents are looking to apply from Canada. Ideally would like to stay around 4-5 months. What are the best options? SETV for 90 days and then extend for another 90? METV for 6 months? METV needs $15k in a bank account?

They are willing to leave the country and return.

TLDR : Answer Summary
The best visa options for retired parents from Canada wishing to stay in Thailand for 4-5 months include the Single Entry Tourist Visa (SETV) for 90 days, which can be extended for an additional 30 days, and the Multiple Entry Tourist Visa (METV) that allows for stays of 6 months, requiring a border run for re-entry. The METV requires a minimum of $15,000 in a bank account. For a more suitable option, they might consider applying for a retirement visa with adequate income documentation, which could allow for a longer stay.

Go the extension based on retirement. Email the Canadian embassy for a verification letter of income. $50.00 If they make over 65 K Thai Bhat a month.

They can apply for the extension while in Thailand. They can do it themselves. 1900 Bhat for each extension.

You can't extend a tourist visa for 90 days. Tourist visa is for 60 days and can be extended one time for 30 more days. You'd need to leave the country and re-enter to get more time, even with multi entry.

If they don't get a retirement visa then a stay of 6 months will require leaving Thailand once to get stamped in. Are they able to do this?

Non-O or Non-OA. But a non-O would require getting an extension or leaving the country.
