Do I need a mobile notary or an onsite notary to notarize my doctor's signature for a medical certificate from the Chicago embassy?

This the medical certificate downloaded from the Chicago embassy. It certainly appears to require that it is the doctor’s signature that requires notarization. Does this mean I need to employ a mobile notary to meet me at the doctor’s office or get it done at a hospital with an onsite notary?
