TLDR : Answer Summary
The requirement for a COVID-19 test before leaving Thailand generally depends on the entry regulations of your destination country. To leave Thailand, you do not need a COVID test unless your destination requires it. If a test is necessary, it is advised to inquire about testing locations through local social media groups, such as those in Bangkok or Chiang Mai.

To leave Thailand, you need to do the following:

-IF You have an extension of stay:

IF you are planning on returning, to get a re-entry permit from the immigration office. IF you are not returning, to cancel your Extension of Stay and work permit (if it applies to you)

-IF you are currently on amnesty:

You do not have to do anything to leave Thailand.

-IF you are flying to a country where a COVID-19 test is required, and you're asking where to get a COVID-19 test, post this question in a local Facebook group (i.e. in a Bangkok related group, or Chang Mai related group, etc...)

There is no requirement for a COVID test to leave Thailand.

But the country where you are going may require one.
