I used last week Rajavej hospital very quick and efficient result 5 hours later.(I did post about it)

They just wanted me to confirm that the address I was staying out was my home address with some ID they’re absolutely fine to deal with in my case.

But yes you need your gov locator form and the acknowledgment from the Covid test centre with your ref no.
