I just had the gym package receipt, business registration page and copy of the owner’s passport. Also I did it in person before it moved to evisa (but don’t think it makes that much difference)

Hanoi. I got a 6 month package, 48k, but I believe they have other lower cost packages for DTV also

thanks, one last thing - did they provide the certificate from the sports authority saying they are a registered Muay Thai gym. Because reading recent reports some people are being rejected because of that

If you only need the visa Fitness Hero Phuket is doing them, then I guess you free to choose gym.
