Just upload a picture of your passport for both. They shouldn't be mandatory sections to upload data, but Thailand and computers don't do well together.

Pretty sure anyone can enter. Without a vaccine you need a PCR test to enter, with a vaccine an AKC is fine. Let me double check though.

Naaah, you'd be looking anywhere from 10k-60k for a year, depending on the type of school and visa (if you're attending class or just want the visa) and where you are in the country.

Only way to know is to go ask the branch. Each one is different. In Bangkok I always recommend Krugsri, I find them to be the easiest to open accounts with.
